Procedures for Health and Accident Insurance Claims When visiting the hospital, please prepare the following documents:
- Insurance Card
- ID Card / Passport or Government-issued Photo ID
- Please present your Insurance Card and ID Card / Passport at the Registration Counter or Nurse Station.
- Registration staff will verify your insurance details and inform you of your preliminary coverage and conditions.
- After treatment, finance staff will request the patient's signature on the claim form. The patient is responsible for any expenses exceeding the coverage limit (subject to policy conditions).
- If the treatment is not covered or does not meet policy conditions, you will be required to make an advance payment. You can then submit the claim form and receipt to your insurance company for reimbursement.
- Please present your Insurance Card and ID Card / Passport at the Registration Counter or Nurse Station.
- Staff will present the claim documents and a copy of your insurance card for your signature.
- Once the coordination with your insurance company is complete, staff will notify you whether the Fax Claim (Cashless Service) can be processed.
- Upon your discharge, if you are eligible for the Fax Claim service, our Inpatient Finance staff will coordinate with your insurance company. We will submit your medical expenses and the physician’s medical summary for their assessment of your maximum benefits. Should there be any non-covered expenses, the Finance Department will notify you, and any outstanding balance (if any) must be settled before you leave.
- The hospital will provide the claim form and receipt for you to submit to your insurance company for reimbursement.
